Welcome to Health Challenge Pembrokeshire, our second Health, Social Care and Well-being Strategy for 2008 to 2011. Pembrokeshire County Council and the Pembrokeshire Local Health Board have worked together with the NHS Trust, the voluntary sector, the National Public Health Service, the local Community Health Council together with local communities and the public to develop this strategy.
Improving health, social care and well being is not something any one organisation can do alone. That’s why, in this strategy, many organisations have come together, as partners, to take joint action. As individuals we also have a part to play, and we are being encouraged to take a greater responsibility for our own health. The strategy emphasises the importance of looking after our own health and well-being and encourages us to lead a healthier lifestyle.
We are working to establish a two-pronged approach to improving our health and well-being: first by preventing ill health in the first place and secondly, by improving Pembrokeshire’s health and social care provision for residents who need these services.
We will continue to work with our partners to tackle the root causes of ill health by identifying the risk factors that affect our health. We will encourage people to take more responsibility for their own health by raising health awareness and look to deliver more services in the community to support independence. We will continue to work towards reducing inequalities between communities and work with our partners to target services to vulnerable people who
have additional health and well-being needs.
